FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. Inspected two vehicles – Tag#YA138377, VIN# correct per the firm application & Tag#KT-5489, VIN#1HTMNAAM22H54669, 400 waste, which was not listed on the firm application. Ensure that the new application submitted for the firm renewal this winter has the correct info for both vehicles. The tanks on both vehicles appeared structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted. The lettering was correct on both sides of both vehicles per the info on the firm application. 2. Reviewed the truck logs and it appears that the required info is being maintained at this time. 3. Mr. Hunt stated that he plans to attend the continuing education class in Raleigh in December. 4.
