with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected vehicle Tag#YA142207, VIN# correct per the firm application. The tank appeared structurally sound
& securely attached. Outlet covers were on and no drips/leaks were noted. The lettering is correct on both sides
of the vehicle per the info on the firm application.
2. The other vehicle listed on the firm application was not available for inspection at this time. Mr. Metcalf stated
that the vehicle was not currently in service.
3. Truck logs were not available for review at this time. Enclosed is an example truck pumping log. Ensure that
the required information is being maintained for every septage pump job that is performed by this firm.
4. The firm renewal process is currently in progress. Mr. Metcalf stated that he had attended a 2021 continuing
education class.
