Expires 12/23/2057. PURPOSE OF SITE VISIT: Partial Facility Compliance Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 7 ADDITIONAL COMMENTS: 01 - This facility was issued a Life of Site permit on August 15, 2015 and expires on October 11, 2057. 02 - Facility has adequate signage. 03 - Facility is secured by chain link fences and locking gates. 04 - Interior roads are made with all-weather construction. 05 - Waste from this facility is permitted to be taken to Sampson Co. Disposal 8202-MSWLF-2000, East Carolina Regional Landfill, 0803-MSWLF-1993 and Brunswick Waste Management Facility, SWP 583 in Lawrenceville Va. 06 – This facility is designed to handle up to 1,000 tons per day. 07 - Facility is permitted to receive waste from within, Duplin, Edgecombe, Franklin, Greene, Halifax, Johnston, Lenoir, Martin, Nash, North Hampton, Pitt, Wake, Wayne and Wilson Counties 08 - The Leachate drains at the Transfer Station were flowing into the leachate system during this inspection. One area is in need of cleaning. 09 - The tipping floor was at low volume during this inspection. 10 - Facility has several certified operators: Ricky Knight, issued, 7/24/2021 and Andrew Ward, expires, 6/21/2022. 11 - Screening records were reviewed and up to date. 12 - Load tickets were not reviewed during this inspection. 13 - No leaking trailers were noted during this inspection at the stored trailer area. Need to rake up OLD small waste from this area by the end of each working day. 14 - There was minimal windblown trash during this inspection. 15 - There is a lot of damage to the south side of the transfer station wall. It appears that the waste has been piled very high inside and has been pushed into the metal walls and broken the screws and bent the exterior metal to the point waste is lodged in these areas with some falling to the ground. Needs to be repaired quickly. 16 - Some of the falling waste (smalls) appeared to have been there longer than today. This area MUST be cleaned up by the end of each working day along with any other windblown waste. 17 - John College (DWM) met with Michael Lee (GFL Operations Manager) after this inspection at the GFL office to discuss issues that were found and would be added to this report. 18 - Digital pictures were taken during this inspection and some are attached to this report.
