SITE PLAN
James Patterson, Waste Management Specialist with the N.
Department of Environment, Health, and Natural Resources from
Hayesville, N.C. has visited the site and reviewed this appli-
cation.
The Aerial Photograph enclosed is hand -drawn, because I could
only get one copy of the photo and it was illegible.
This site is located on State Road 1107 (East Fork Road), approx-
imately nine miles south of Brevard off of State Highway 276.
I have leased approximately 1.5 acres from L.R. & Mary J. Hebson,
(Deed Reference: Deed Book 333, Page 31).
The terrain is steeply sloped woodland, with a gently sloping
entrance to the State Road 1107. The State Road on the west
side of the site is the head of the watershed so no drainage
from the site can flow toward the adjoining development. Sur-
face water from SR1107 can be diverted off of site by means of
the old logging road and a ditch along SR1107. A settling basin
will be built at the northeast corner of the site, approximately
2000 cubic feet in size, to retain all sediment on site. The
sediment basin will have a brush barrier below it to catch any
sediment from sediment basin until grass is established.
No demolition material will be dumped within 50 feet of the
spring located at the northeast corner of the site. The sediment
basin will be kept cleaned out during the filling of the land-
fill. Seeding will occur as the slope is filled with demoli-
tion materials and covered with soil to prevent erosion. The
seeding schedule is enclosed with the erosion control plan.
Any fill required will be used from on -site, from up on slopes,
away from any ground water.

Dear Mr. Cantrell:
The Solid Waste Section has conducted a preliminary review of
the above referenced application for a demolition landfill permit.
In accordance with the North Carolina Solid Waste Management Rules,
the following comments must be addressed to continue the review
process:
1) In accordance with .0503 (2) (d) (ii) , "A site shall be designed
so that the bottom elevation of solid waste will be a minimum
of four feet above seasonal high groundwater."
As a spring head is located on the northeastern side of the
property to be disturbed, the depth to groundwater on this
site needs to be determined prior to development.
2) In accordance with .0506(2)(a), the "proposed filling limits'
need to be shown on a site plan. The only detail on this was
shown on the northeastern side of the property next to the
spring head. The areas to be filled need to be clearly shown
on the map.
Furthermore, proposed filling limits means that the applicant
must show the proposed excavation and final contours for the
site. * Proposed excavation depths need to be four feet above
groundwater. A benchmark may be used to locate elevations.
In addition, two cross-section of the fill area showing the
proposed excavation and final contours may enhance the
application.

This office has reviewed the erosion and sedimentation control plan
submitted for the project listed below. We find the plan to be acceptable and
hereby issue this Letter of Approval. Please be advised that Title 15A, North
Carolina Administrative Code 4B.0017(a), requires that a copy of the approved
soil erosion control plan be on file at the job site. Also, you should consider
this letter to give the Notice required by NCGS 113A-61(d) of our right of
periodic inspection to ensure compliance with the approved plan.
The State's Sedimentation Pollution Control Program is a performance -
oriented program requiring protection of the natural resources and adjoining
properties. If, following commencement of this project, it is determined that
the plan is inadequate to meet the requirements of NCGS 113A-51 to 66, this
office may require revisions to the plan and implementation of the revisions to
ensure compliance with the Act.
Please note that this approval is based in part on the accuracy of the
information provided in the Financial Responsibility Form which you have
provided. You are requested to file an amended form if there is any change in
the information included on the form. In addition, it would be helpful if you
notify this office of the proposed starting date for this project. Your

Dear Ms. Molloy:
In response to your letter, a copy of which is attached, requesting
more information on our application, please find enclosed a cover
sheet showing two cross sections, A and B showing existing grade and
finish grade. It also shows the location of seven test holes on
cross section sheet A, which indicates ground water is at least
four feet below the bottom elevation of the proposed solid waste.
The test holes were dug by hand with a post hold digger. The ele-
vations were taken using a benchmark from a stake set close to the
spring head at elevation 2992.00.
We hope this clarifies the application.

CONDITIONS OF PERMIT:
1. This permit shall not be effective unless the certified copy is
files in the Register of Deeds Office, in the grantor index
under the name of the owner of the land in the county or
counties in which the land is located. After recordation, the
certified copy shall be returned to the Solid Waste Section and
shall have indicated on it the page and book number, date of
recordation, and Register's seal.
2. When this property is sold, leased, conveyed or transferred, the
deed or other instrument of transfer shall contain in the deed
description section in no smaller type than that used in the
body of the deed or instrument a statement that the property has
been used as a sanitary and.demolition landfill.
3. The following requirements shall be met prior to receiving solid
waste at the site:
a. Site preparation shall be in accordance with the
construction plan.
b. Site inspection shall be made by a representative of the
Division of Solid Waste Management.
C. The permit number and the words "NO UNAUTHORIZED DUMPING
ALLOWED" shall be posted on an entrance sign.
4. This demolition area is permitted to receive solid waste as
defined in 15A NCAC 13B, .0101(5); stumps, limbs, leaves,
concrete, brick, wood, and uncontaminated earth.
5. This permit is for construction according to the attached plans.
Additional conditions and revisions of the approved plans shall
be approved by the North Carolina Solid Waste Section.
6. This facility shall conform to operating procedures in Rule
.0507 of the Solid Waste Management Rules and the following
specific conditions:
a. Solid Waste shall be restricted to the smallest area
possible and compacted as densely as practical. The active
area shall not exceed on (1) acre in size.
b. Adequate soil cover, a minimum of 6-inch layer, shall be
applied monthly, or when the active area reaches one (1)
acre in size.

6. Ground water quality at this facility is subject to the
classification and remedial action provisions of 15 NCAC 2L.
7. All earth disturbing activities will be conducted in accordance

I. GENERAL CONSIDERATIONS
The proposed demolition landfill for Cantrell Construction
Company is located on State Road 1107 (East Fork Road) and will
involve filling a hollow with demolition materials. The total
land area is 1.5 acres, but less than an acre will be disturbed
at any one time. The terrain is steep and woodland. There is
a springhead located on the northeast corner of the property.
Access to the demolition landfill will be from State Road 1107
with a gate to prohibit unauthorized dumping. Planned use of
this site will involve approximately a ten year time period, so
seeding will occur on the slope as it is completed and filled.
The Erosion Control measures utilized on this site are a
2000 s.f. settling basin located at the northeast corner of the
site. Surface water from State Road 1107 will be diverted a-
round the site by means of an old wood road located on the south-
east side of the site. Erosion Control below the sediment basin
will utilize a standard brush barrier.
