violation of the Solid Waste Statute or Regulations. If the violation(s) noted here continue, you may be subject to enforcement actions
including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further
relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:
(1) The damaged metal on the side of the building still needs repairing or replacing.
(2) Recording looks really good.
(3) New tipping floor seems to be holding up well.

OTHER COMMENTS /SUGGESTIONS:
(1) The approval for the temporary transfer facility issued prior to the operation of the permanent transfer facility has
been re -issued. The tipping floor is undergoing repairs and one half of the floor is being utilized for tipping
municipal solid waste while C&D waste is being placed at the temporary area.
(2) Make sure that all waste is removed from the C&D pad each day.
(3) Re -instate waste screening at the temporary C&D area.

RE: Jackson County Transfer Station Slab Repairs

I have attached the revised scope of slab repairs. The new version involves removing approximately
90% of the existing upper slab, and replacing it with a 12" thick slab for the first 12 feet from the
retaining wall and a 6" slab for the remainder of the area. The existing steel `funnel' will be removed
and replaced with a similar version.
Same as before, the work must be done in two concrete placements to allow for continuous operation
of the facility. A temporary concrete barrier must be placed between the construction area and the
operational area. In our site meeting last Thursday, Chad felt that he could arrange to get temporary
traffic barriers from the DOT, please coordinate that with him. The barrier must also restrict the flow
of water from the operational side to the construction side. Talk with Gary Buchanan about some of
the ideas that we discussed at the meeting.

Slab Repair
CONCRETE
1. Concrete for the project shall be designed for the following properties:
9 The design intent is to produce a durable concrete with low shrinkage
characteristics.
Maximum 28 Day Shrinkage Test Results of 0.03% in accordance with the
Modified ASTM C157
• Minimum 28 Day Compressive Strength (f c) = 4000psi
• Cement type: 1
e Flyash: 15%-25% flyash by weight of cement
+ Maximum Water / Cernentitious Material Ratio (W/CTVP: 0.40
• Nominal Size of Coarse Aggregate: 1", non-nal weight
• Admixtures: high range water reducing admixture (super -plasticizer)
! Slump: 2"-4" (prior to adding super -plasticizer) 5"-7" (after adding super -
plasticizer)
+ Surface Fizush: Hard Trowel
• Curing Method and Time: wet cure for 14 days
2. No sawcut joints are required,
3. Allow the concrete of Pour #1 to cure for a mit imutn of 21 days prior to putting it
into service.
VAPOR BARRIER
1. The vapor barrier beneath the slab on grade shall be Moistop, manufactured by
Fortifiber Corporation.
2. place: vapor barrier sheeting in position with longest dimension parallel with
direction of pour_ Lap joints 6 inches and seal with manufacturer's recommended
mastic or pressure -sensitive tape.
