The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected new vehicle/replacement vehicle – tag#YA164045, VIN#1HTMSSTR0GH371417, 4000 gallons.
Tank is structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted. The
lettering (per the info on the firm application) did have an issue with the town name & permit number at less
than three inches but the lettering was corrected this same day & photos sent – based on these photos it appears
that the lettering is now in compliance on both sides of this vehicle. Thank you for correcting the lettering.
2. Reviewed the truck pumping log – the example truck log is being used. Upon discussion of the logs it was noted
that a daily total for jobs was being recorded, not the required information for each separate pumping event. Per
an email the firm stated that changes have been made to record the info correctly moving forward.
3. Mr. Stevens stated he had attended a continuing education class this year. The firm permit renewal process is
currently under way. Please ensure that authorization forms for any WWTP that may be used in 2022 are
submitted with the firm renewal application.
