STATUS OF PERMIT:
Permit to Construct Phases VI thru X and Operate Phases 1 thru V, issued September 9, 2009 expires September 16, 2014; PTC/PTO LCIDLF, Phases I through X issued April 20, 2015 and Expired April 20, 2020. Renewal has been submitted.
PURPOSE OF SITE VISIT: Comprehensive Inspection
STATUS OF PAST NOTED VIOLATIONS: NA OBSERVED VIOLATIONS: NA
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS 1. The purpose of this site visit was to conduct a comprehensive facility inspection. Permit was expired at time of inspection. Renewal was submitted after last inspection. Follow up with Ben Jackson, Permitting Engineer, on progress and if anything is needed to finalize the renewal. Contact Information Section Permitting Branch Engineer Benjamin Jackson – Benjamin.Jackson@ncdenr.gov / 919 -707 – 8234 office. 2. Excellent sign at entrance to facility. Facility is still gated. Current access road, if maintained properly, should continue to meet rule requirement. New rule siting requirements are that an access road around the EOW be
installed and maintained.
15A NCAC 13B .0566 (7) - Access roads shall be of all-weather construction and shall be maintained to
allow access by vehicles transporting waste, Department staff, and fire-fighting vehicles. 15A NCAC 13B .0564 (7) The site shall establish and maintain an access road around the waste boundary for access by emergency or fire-fighting vehicles and equipment. 3. No unacceptable waste was observed at the time of this inspection. 4. Edge of waste markers were clearly visible. 5. Slopes looks good. 6. Waste in older cells seemed covered well and no standing water. 7. Vegetation established on LCID, looks good. Site needs mowing soon so that shrubs and trees don’t return.
