Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. Page 1 of 3 FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management NORTH CAROLINA Solid Waste Section ADDITIONAL COMMENTS 1. Photos included in this report were taken by Susan Heim on October 6, 2021 during this inspection. 2. This facility serves as the yard waste treatment and processing site for Forsyth County. The facility is permitted to receive wood waste, including yard waste; unpainted and untreated wood waste and wood pallets; woody waste from disaster debris management and land clearing activities; and, leaves and grass clippings. 3. Composting is no longer a permitted activity at this facility, effective with the treatment and processing permit issued October 19, 2015. 4. The City of Winston-Salem contracts with Wallace Farms, Inc. to manage operations at the facility. Wallace Farms provides a site supervisor who serves as the facility's operations manager and is on duty during operating hours. A City/County Utilities staff member is also onsite during operating hours and serves as the scale house operator. 5. Copies of the facility Permit to Operate and the approved Operations Plan are kept on file in the scale house and are available for review upon request. 6. A review of the facility annual report for FY 2020-21 indicated that materials received at the facility totaled 30,310.65 tons. 7. Mr. Rickett provided tonnage records of waste received at the facility during the current fiscal year via email following the inspection. Monthly tonnage was recorded as follows: July 2021 — 2,167.64 tons; August 2021 — 2,496.99 tons. Figures for September 2021 had not yet been compiled for the facility operating record. 8. The site is surrounded by a chain link fence and is secured by two locking gates which prevent unauthorized access. The entry gate is located in the northwest corner of the site. The second gate is located in the southeast corner of the site and is used, only when needed, for tractor trailer access. 9. Proper signage was observed at the entry gate. Inside the facility, brightly colored informational/directional signs are strategically placed to assist customers in finding the correct location for unloading specific materials. 10. The facility was observed to be clean and well -maintained. 11. All incoming and outgoing loads are weighed and recorded. Preliminary waste screening takes place at the scale house, where the operator uses a closed-circuit camera system to inspect incoming loads prior to directing them to the appropriate tipping location. 12. The entire site is paved, and access roadways were found to be in good condition. However, the areas of broken and missing pavement that were noted in the operations areas during the previous inspection on October 30, 2020 appeared to have increased in size. Mr. Rickett stated that stone is applied to the worn areas as needed to maintain the operations pad. No ponding water was observed in or around the piles of wood waste. Mr. Rickett stated that bids had been received for repaving the operations areas, but prices had been higher than anticipated. He added that City/County Utilities staff are currently working on a plan to incorporate the project into the operating budget. 13. A roll -off container is used to collect unacceptable waste that is separated from incoming loads. The container is taken to Hanes Mill Landfill (Permit # 3402) as necessary to properly dispose of the waste. 14. Grinding operations were underway during this inspection. An excavator was feeding wood waste into a large tub grinder, while two loaders were working to ferry the unprocessed material to the operations area and to stockpile the processed material as it left the grinder. 15. Please ensure that the ground wood waste stockpile meets the size restrictions of 30' high and 50' wide, as set forth in the current, approved Operation Plan. 16. Processed wood waste is loaded into trailers and transported to the Wallace Farms facility in Davie County (permit # 30-04) for further processing and composting. 17. Mr. Rickett stated that the permanently installed horizontal grinder on the east side of the site is not currently in use. 18. Fire extinguishers are installed in each piece of equipment that operates at the facility, and water is accessible through a hose bib located in the operations area. 19. Storm water disperses and sheet flows across the site. An asphalt berm, situated near the top of the driveway leading to the back (southeastern) gate, slows the flow of surface water and directs it to the grassy area along the southern property line. No evidence of erosion or sedimentation was visible during this inspection. 20. The facility appeared to be in compliance with required setbacks. Uncontaminated wood pallets are separated from other wood waste and stockpiled near the southeastern corner of the property. 22. No unacceptable waste was observed at the facility.
