Inspections of solid waste transfer facility, scrap tire collection site, and yard waste T and P.
NOTICE OF VIOLATION(S) (citation and explanation):
NONE
You are hereby advised that, pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $5,000 per day may be assessed for each
violation of the Solid Waste Statute or Regulations. If the violation(s) noted here continue, you may be subject to enforcement actions
including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further
relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:

1. The leachate collection tank is full and needs to be pumped.

2. A few small pieces of pegboard were observed at the yard waste area. Remove this material.
3. The walls inside the transfer building are being cleaned.
4. Tipping floor looks good. Washdown adequate.
5. Roger Smith and Larry Hogsed have attended the SW ANA transfer operators training. Roger attended the training
November 15 and 16, 2005 and Larry attended the training May 15 and 16, 2006.
6. The scrap tire collection area looked good. Tires were being properly stacked and no tires were observed outside the
trailer.
7. White goods area looked good. Appliances with cfc's are being seperated from other materials and properly stored.
8. Operation is still improving. Keep up the good work.

AREAS OF CONCERN AND COMMENTS:
1. Materials accumulated on the inside of the push walls (metal, etc.) need removing.
-, Leachate holding tank was almost full at the time of the inspection. I would suggest checking the liquid levels in the
tank every 2 or 3 days and document findings.
3. Sn1all an1ounts of trash observed in the pit area and in the rip-rap ditch line. Clean up this refuse.
4. 11 is time to wash down the interior walls of the facility.
S. The back wall at the metal storage pad needs repairing.
6. Service area for this facility is Clay County.
7. Roger Smith and Lany Hogsed have attended the SW ANA Operators training. Roger attended the training November
IS and 16, 2005 and Lany attended the training May 15 and 16, 2006.
