STATUS OF PERMIT:
640 1 -MSWLF- 1983 (Closed): The MSWLF ceased accepting waste during the summer of 1998, closure was documented
in a CQAR prepared by GEI Consultants and dated December 1998.
6401-CDLF-1999 (Closed): Locate on top of the closed MSWLF landfill (6401-MSWLF-1983), construction and
demolition waste was accepted until 2000, final closure was documented in a CQAR prepared by GEI Consultants and dated
March 2000.
6403-CDFL-2000: Permit to Construct Phases 6-9 and Permit to Operate Phases 1-5 was issued August 31, 2021
PURPOSE OF SITE VISIT:
Comprehensive Compliance Inspection
STATUS OF PAST NOTED AND OBSERVED VIOLATIONS
A. 15A NCACI313.1626 (8)(b), "Surface water shall not be impounded over or in waste."
RESOLVED: The County has cut the side slope berms of 6401-MSWLF-1983 in several places to help alleviate the
previously identified areas of standing water over waste. No standing water was observed near the cuts. Mr. Barnes
stated that he would have the areas cut with a weed eater and additional gravel would be added to help control erosion.
Continue to monitor the low areas of 640 1 -MSWLF- 1983 to ensure that there is no water impounded over waste.

B. 15A NCAC 13B .0542 (k) (1 and 2), "Adequate sediment control measures consisting of vegetative cover,
materials, structures or devices must be utilized to prevent sediment from leaving the C&DLF facility... (and)
prevent excessive on -site erosion of the C&DLF facility or unit."
RESOLVED: The sediment basin, located to the north of 6403-CDLF-2000 (Phase 4), has been cleaned out and new
baffles installed. The side slopes of the basin have also been seeded and covered with straw. The side slopes need to be
kept free of woody vegetation and repaired/seeded as needed. Continue to monitor the depth of sediment in the basin
and clean out the basin as needed. Filter stone and riprap have been installed around the stormwater inlet. Please contact
DEMLR if there are additional questions regarding the inlet to the sediment basin.

OBSERVED VIOLATIONS
None.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
ADDITIONAL COMMENTS:
1. Edge of waste (EOW) markers were observed around 6401-MSWLF-1983, 6401-CDLF-1999, and 6403-CDLF-
2000. Continue to mow around the EOW markers so that they are visible from one to the next.
2. The grasses were becoming overgrown on 6401-MSWLF-1983 and 640 1 -CDLF- 1999. The overgrown vegetation
should be mown to allow for proper inspection of the site.
3. Two areas of ATV tracks were observed up the side slopes and on top of 640 1 -MSWLF- 1983. It appears that the
ATVs are using the powerline to access the property. Mr. Barnes stated that there are also several additional possible
access points in the surrounding forest. No trespassing signs have been posted, which are not deterring the ATV
riders. During the inspection, it was recommended that the potential access points be blocked or utilizing the Sheriff

to issue trespassing tickets.

4. The monitoring and gas probe wells were observed around 6401-MSWLF-1983 and 3401-CDLF-1999. Overgrown
vegetation was observed around a majority of the wells. Several of the wells had damaged protective casings and/or
broken concrete. Wells should be constructed and maintained in accordance with 15A NCAC 02C.
During the inspection, it was recommended that during the sampling events, the sampler document the condition of
the wells on the sampling form or a separate form. This documentation would help keep track of the need for repairs
and completed repairs. The vegetation around the wells needs to be removed and the damaged wells repaired.

6403-CDLF-2000 Phase 4 had reached capacity and is no longer being used. Some areas of exposed waste were
observed along the side slopes. Portions of Phase 4 had been seeded, however, the most recently completed portion
had not been seeded. Ms. Thompson reminded Mr. Barnes that all waste needs to be covered and the area stabilized
and seeded to prevent erosion.

6. On August 31, 2021, the Section issued the Permit to Operate for Phase 5. 6403-CDLF-2000 Phase 5 was in use
during this inspection. Mr. Barnes stated that the compactor and one bulldozer were down due to mechanical issues.
The mechanic was on site during the inspection to repair the equipment. No standing water was observed in Phase
5. A temporary berm was observed in Phase 5 to separate possible leachate from stormwater. Any leachate
generated during the operation of Phase 5 will need to be pumped and hauled to a wastewater treatment
facility.

The metal recycling pile has been reduced since the last inspection. A wood frame with paper was observed in the
metal pile. Mr. Barnes stated that the wood frame likely came from a recently demolished mobile home. Mr. Barnes
was reminded that the metal recycling pile was for metal, not wood. Mr. Barnes stated that he would have the wood
frame removed from the metal pile.

8. The slope drain along the western side of the inactive CDLF, 6401-CDLF-1999 had been damaged and was found
in pieces. The slope drain should be repaired or removed if no longer needed.

9. The two sediment basins located to the west and north of the inactive CDLF were observed during this inspection.
The basins were identified to be grassed with some woody vegetation along the side slopes. No standing water was
observed in either basin. Based on conversations with Thad Valentine, Environmental Senior Specialist with the
Division of Energy, Mineral and Land Resources (DEMLR) and a review of the submitted erosion and
sedimentation (E&S) control plan, the basins are not part of the current active CDLF E&S plan. The basins would
be utilized during future expansion and tie-ins of the CDLF. It is recommended that the basins remain for future
use. It is also recommended that the basins be mowed, and woody vegetation be removed from the side slopes to
maintain the integrity of the basins.

10. The permit, operations plan, and records were review during this inspection. The cover log for the CDLF was not
up to date. The last recorded cover was August 24, 2021. Ensure that the cover log is updated every time cover
is applied.
