The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Proper signage was observed at the entrance of the facility.
2. The facility is permitted to receive solid waste within approximately 100 miles around the landfill.
3. Edge of waste markers were visible.
4. The working face was small and compact.
5. The facility is currently working in Phase 2B.
6. The facility had pulled a tire and plastic barrel out of the working face and set aside to dispose of properly. No
other unacceptable waste was observed.
7. A "roll off container" is staged near the working face for the collection of scrap tires.
8. All access roads maintained in good condition and the facility applies water to keep the dust down.
9. The facility is scheduled to mow the inactive areas in October.
10. Partial closure plan was approved November 20, 2019 identifying the area of the landfill to receive the closure final
cover system, which is approximately 4.26 areas. Permanent vegetative cover was observed along the north side of
the landfill in this area.

Semi-annual groundwater monitoring report dated February 2020, 1st Semi-annual groundwater monitoring report
dated July 2020, 2nd Semi-annual groundwater monitoring report dated February 2021, 1 st Semi-annual groundwater
monitoring report dated September 2021. Surface water monitoring is performed semi-annually with their
groundwater monitoring. All reports were reviewed on the divisions database.
12. Quarterly landfill gas monitoring is performed by Blue Flame LLC. 1' Quarter 2018, 2nd Quarter 2018, 3rd Quarter
2018, 4th Quarter 2018, 1st Quarter 2019, 2nd Quarter 2019, 3rd Quarter 2019, 4th Quarter 2019, 1st Quarter 2020, 2nd
Quarter 2020, 3rd Quarter 2020, 4th Quarter 2020, 1st Quarter 2021, 2nd Quarter 2021, 3rd Quarter 2021 were
reviewed. Mr. Geiss provided these records via e-mail.

14. Mr. Geiss provided random inspection reports via e-mail. Facility staff perform one random inspection each day.
Reviewed reports from January 2021 to September 2021. Good documentation of unacceptable waste found with
documentation of final disposition.
Certified Operators are as followed:
Jeffrey Geiss — Certified Manager of Landfill Operations expires 2/11/2022
Jeannette Ramos — Certified Landfill Operations Specialist expires 2/10/2024
