or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected vehicles tag#YA151104 & tag#YA126977, VIN#s correct per the info on the application.
The tanks on both vehicles appear structurally sound & securely attached. Outlet covers were on & no
drips/leaks were noted. The lettering is correct on both sides of both vehicles per the info on the firm
application. However, vehicle tag#YA151104 did have some cracked/damaged decals which are still legible at
this time but will need replaced in the future.
2. Reviewed a combo truck & site log – it appears that the required info is being maintained. Ms. Cooper stated
that she would be attending a continuing education class in the coming months to obtain the required 4 hrs of
firm training and 3 hrs of land app training. The Raleigh Central office has recently sent out notifications in
regards to the firm permit renewal to be done this winter – if you do not receive this notification please contact
the Raleigh Central office.
