The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS:
01 - The Permit to Operate shall expire April 22, 2030. Pursuant to 15A NCAC 13B .1401 (c), no later than December 22, 2029, the permittee must submit a request to the Section for permit review and must update pertinent facility plans.
02 - The facility must have a trained operator, in accordance with 15A NCAC 13B .1409 (19), no later than November 1, 2022. A responsible individual trained in facility operations must be onsite during all operating hours of the facility. John College emailed R.V. Knight web link to access information on 9/27/2021. 03 - Compost Windrow sizes appear not exceed maximum size of 10ft high and 22ft wide. 04 - Stockpiled feedstock windrows appear not exceed maximum size of 30ft high and 50ft wide. 05 - Facility has required signage.
06 - Facility is secured by fencing and locking gates. 07 - Interior roadways are constructed off all-weather materials. 08 - Fire lanes are present. Per permit conditions.
09 - Facility has fire extinguishers available in working areas and the office to fight fires. 10 - Local Fire Department has keys to facility entrance gate to enable quick entry for any fire that may occur after work hours.
11 - All on-site feedstock materials were of the permitted types. 12 - Facility is currently operating under the new permit and appears to be in compliance. 13 - No leachate was noted leaving site footprint. 14 - Incoming feedstock records were not reviewed during this inspection. 15 - Outgoing load records were not reviewed during this inspection. 16 - Temperature records were reviewed and were up to date. 17 - Temperature probe calibration records reviewed and up to date. 18 - Non conforming waste is pulled out and put into a Republic Services dumpster. 19 - Overall conditions at this facility were good. 20 - Digital pictures were taken during this inspection and some are attached to this report.
