Hilltop Road Red Springs, NC 28377 PARTICIPANTS: Connie Wylie, Emily Cooper STATUS OF PERMIT: Active – permit expires July 7, 2022 PURPOSE OF SITE VISIT: Routine STATUS OF PAST NOTED VIOLATIONS: Situation has not improved – see observed violation below. OBSERVED VIOLATIONS: 1. .0842(a)(7) Septage shall be applied to the surface of the land from a moving vehicle to have no standing liquid or soil disturbance resulting from the waste flow after the discharge is complete. Noted various areas of heavy application & damaged cropstand on site. This was also noted on the previous audit on 4/26/2021. At this time the damaged areas will be repaired with an adequate establishment of the winter cover crop. But corrective actions need to be implemented to prevent heavy application on the crop this winter or in the future. SEE THE NEXT PAGE The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. It appears that the damaged field markers have been reposted. Markers appear adequate. 2. No sign of land application outside the permitted boundaries. It appears that the approved setbacks are being met but were not measured at this time. The site has been recently used/currently in use. No odors noted. 3. Hay bales are currently stored along the field perimeter and need to be removed in a timely fashion. The winter cover crop shall be sufficiently established as per the approved nutrient management plan for land application to occur this winter. 4. The required signs are posted and the access road is adequate. 5. If any firm other than Cooper Septic Tank Service will be utilizing this disposal field then an authorization form shall be submitted yearly and also a copy of the site logs shall be submitted monthly. 6.
