The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected three vehicles - tag#YA-46424, tag#YA-141736 & tag#YA-133647. VIN#s correct for all three
vehicles per the firm application. The tanks on all three vehicles appear structurally sound & securely attached.
Outlet covers were on and no drips/leaks were noted on any of the vehicles. The lettering is correct on both
sides of all three vehicles per info on the firm application.
2. Reviewed the truck pumping logs and it appears that the required information is being maintained. Ensure that
use of the SDTF is recorded correctly in the truck pumping logs and also the name of the specific WWTP when
utilized.
3. The required continuing education hours (4 hrs for firms & 3 hrs for SLAS) have not yet been obtained –
discussed a possible class location this fall. Discussed that the Raleigh Central office has recently sent out the
notifications regarding the firm permit renewals this winter – contact the Raleigh Central office if you do not
receive the notification.
