Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. Inspected vehicle tag#YA-79064, VIN# correct per the firm application. The tank appears structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted. The lettering is correct on both sides of this vehicle per info on the firm application. The phone number is faded on both sides, but is still legible at this time. 2. Reviewed the truck pumping logs and it appears that the required information is being maintained. Ensure that use of the SDTF is recorded correctly in the truck pumping logs and also the name of the specific WWTP when utilized. 3. The required continuing education hours (4 hrs for firms & 3 hrs for SLAS) has not yet been obtained – discussed a possible class location this fall. Discussed that the Raleigh Central office has recently sent out the notifications regarding the firm permit renewals this winter – contact the Raleigh Central office if you do not receive the notification. If this pump truck may go to the Siler City WWTP then submit a Siler City WWTP authorization form (along with the Asheville WWTP authorization form) for the firm renewal this winter. Also discussed that possibly this firm permit may not be renewed in the future – submit a firm cease operation form to the Raleigh Central office if this occurs. 4. Thank you for your time.
