PURPOSE OF SITE VISIT: Routine STATUS OF PAST NOTED VIOLATIONS:
No violations noted on the previous inspection report.
OBSERVED VIOLATIONS:
NONE
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1) The required signage was present at Field 2 before you enter the site.
2) The access road was in good shape.
3) Required markers were present. Markers were up and visible on all three fields.
4) No septage discharges noticed outside of the permitted boundaries of the fields, in the field borders, or
along the access road to the fields.
5) No application issues noticed. No spot dumps, thick applications, or grease burns noticed.
6) When I arrived at the site, Marlon was applying septage to Field 2. No issues noticed with the
application. Marlon tested the pH of the septage while I was there, and the septage pH was at 12.

7) Fescue present in all three fields.
8) No objectionable odors noticed today.
9) I received a copy of the soil test report taken last fall. Thank you.
10) Talked to Marlon about the status for submitting the application for permit renewal. Permit set to on
07/01/2021.
11) Setback distances appear to be as permitted.
