PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. The facility is fenced and is secured by a locking gate when it is not open for operation. 2. Correct signage is at the entrance of the facility listing the facility name, permit number, information as well as materials accepted at the facility. 3. The facility accepts waste Monday through Friday from 0700 to 1600. 4. Training records were not reviewed during this inspection. emergency contact 5. Some waste was observed underneath the compactor during this inspection. The on -site attendant stated that the waste would be removed by the end of the day. 6. Windblown waste was being well -managed around the transfer station. Windblown was actively being picked up during this inspection. 7. The damaged curbing that was identified during the February 2021 inspection had been repaired. 8. During the inspection, a large hole was identified in the bottom of the trailer in the trailer loading area. Leachate was dripping from the hole and waste was leaving the hole during the loading of the trailer. The on -site attendant was informed of the hole in the trailer, and he stopped compacting waste in the trailer. The empty trailer on site, was found to have a large hole in the sidewall of the trailer. The attendant ceased accepting waste at the transfer station until the trailers could be repaired. Mr. Brothers contacted GFL Environmental and Hilco to have the trailers repaired. Mr. Randy Deming, GFL Environmental, sent Ms. Thompson photographs of the temporary repairs to the trailers. Mr. Deming stated that the trailers would be taken to Hilco for permanent repairs. Ensure that the trailers are inspected for deficiencies and repaired as needed. 9. The white goods are stored upright on a concrete pad to the north of the transfer station. 10. Scrap tires were laced within a storage trailer. 11.
