Here is the page that was missing in my 2021 report I sent to Raleigh.

I, Ronnie Oaks, dba: Liquid Waste Inc, permit number 04-05 compost 2014.
Teresa I have no choice but to put my compost permit inactive for personal
reasons until Feburary 2022 due to wifes cancer.
I will continue paying the annual fee.
