The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The front corner & back corner posts on the right side of the Right Field are partly hidden by excess vegetation
– remove the excess vegetation. The front corner & back corner posts on the left side of the Left Field are
missing – re-establish these two posts in accordance with the 50 foot setback from the neighboring
property line. The required field posts designating the middle dividing line between the left & right fields are
present. The approved setbacks were not measured but appear to be present at this time.
2. Measured a 500 foot setback from the house that has been installed in the adjacent field in front of the site
(same landowner for that property & this permitted site) – I placed orange flags which need to be replaced
with permanent markers designating the new front line for the disposal fields. The required 500 foot
setback from this residence to the disposal fields shall be maintained.
3. The required signs are posted and the access road is adequate.
4. No sign of land application outside the permitted boundaries. No sign of heavy or recent application. No odors.
5. The cropstand appears adequate at this time. Establish the cover crop per the approved nutrient management
plan so that land application may occur this winter.
6. Logs were reviewed at this time (a combination truck pumping log & land application site log is in use) – it
appears that the required info is being maintained, but ensure that the specific field that a discharge event occurs
on is clearly designated. Dillon stated that he had attended a continuing education class earlier this year.
