The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Discussed the inspection process, permit & rules with Mr. Ivey. Reviewed the site map & setback requirements.
2. A missing field post was noted at the front corner of Field 3 (cut piece is on ground). Reinstall this post at
least 50 feet from the neighboring property line to the north. The other field posts appear accurate. It
appears that the approved setbacks are being met but were not measured at this time.
3. Noted what appears to be a spray path of dead vegetation on field 1. Since only effluent (diluted waste) goes on
the site then the damage would not be from inadequately diluted grease application. Discussed with Mr. Ivey
what fits the definition of septage & can be land applied to this site which is domestic, portable toilet or grease
septage. Ensure that no other waste types are being handled (stored, processed or discharged at this site). This
damaged area will be repaired with the planting of the cover crop per the approved nutrient
management plan to allow land application this winter.
4. No sign of land application outside the permitted boundaries. No recent or heavy applications noted.
5. Ensure that the required continuing education hours were obtained this year (3 hours for land app & 4 hours for
firm training). If not please register for one of the remaining class locations.
6. The land application site logs were not reviewed at this time.
7. The required signs are posted and the access road is adequate.
8. This office received a new soil analysis report in May which included the heavy metals analysis.
