V. PHOTOGRAPHS
Photo 1:
View of CUPOLEX® forms installed on eastern portion of structure.
Onsite to observe VIMS installation activities. Met with Alexis McKenzie (H&H) who
discussed installation of CUPOLEX® aerated floor system. CUPOLEX® appeared to be
approximately 75% installed at time of inspection. Discussed proposed VIMS operation
(active), along with other system details. A single 6” dia. steel riser (to be installed on
northeast side of building) will vent CUPOLEX® VIMS.

CUPOLEX® with Benton Stop® forms (sealed ends) installed adjacent to building footer. Voids
between forms will be filled with concrete once floor slab is poured.
