OBSERVED VIOLATIONS:
1. Rule .0842(a)(17) Approved nutrient management plans shall be followed. Observed a loose cow on Field 2
during this visit – cattle were currently located on Field 1. Cattle shall be controlled on this site to ensure that
the 30 day wait after the last septage application (before grazing) is being maintained in accordance with
the approved nutrient management plan. This is a Rule requirement and a safety/health issue. Discussed the
situation with Terry Jones by phone and with Frankie Jones on site.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. A discharge event occurred on Field 3 during this visit. The pH was checked in various locations and was a pH
12. The spray path was evenly & thinly discharged – a good discharge event. Older areas of heavy/matted
application were observed on this field – ensure that procedures are followed to prevent spot dumping or heavy
application.
2. Overall the cropstand appears adequate on this site with no significant weed problems. Field markers appear
accurate. Setback were not measured at this time but it appears that the approved setbacks are present. No
apparent sign of land application outside the permitted boundaries. No significant odors noted. The required
signs are posted and the access road is adequate.
3. A new house is under construction to the northwest of Field 3, measured the distance at over the required 500
foot setback to the field.
4. Terry stated on the phone that he would sending more site logs soon and also that the site would harvested.
Follow the approved nutrient management plan in establishing an adequate cover crop to allow land application
to occur this winter.
