Please find enclosed a report summarizing the environmental assessment activities in the
area of the existing heating oil UST at the above referenced residence. The site activities
consisted of the identification of the tank location, the collection of a soil sample from
immediately adjacent to the UST, a receptor survey completed by a Licensed Geologist
from Paragon, and the supervision of the in -place abandonment of the UST by a Licensed
Geologist from Paragon. The results of our findings are summarized herein.
Paragon Environmental Consultants, Inc. performed the site inspection at 407 S. Ridge
Avenue on September 11, 2020. A heating oil tank, estimated to be approximately 1,000
gallons in capacity, is located on the south side of the house structure. A geologist from
Paragon obtained a soil sample immediately adjacent to the west end of the tank at the
subject site. A hand auger boring was conducted on this end of the heating oil tank to a
depth of approximately 7 feet below the land surface. The hand auger boring was advanced
to approximately 18 inches beneath the depth of the bottom of the UST as determined from
measuring the burial depth to the top of the tank. Suspected soil contamination was
observed from approximately 5 1/2 feet to the termination depth of the hand auger boring.
The soil sample from the hand auger boring, labeled as T 1-7 to represent the tank and the
its depth beneath the land surface, was submitted to Meritech, Inc. for analysis by EPA
Method 3550. This analytical method tests for high boiling -point hydrocarbons such as
diesel fuel, kerosene, and heating oil. The soil sample indicated a total petroleum
hydrocarbon (TPH) concentration of 154 milligrams per kilogram (mg/kg) which is in
excess of the NCDEQ action level of 100 mg/kg. A copy of the laboratory analytical report
and chain -of -custody record for this soil sample obtained at the subject site is contained as
Attachment A.

A supply well survey has been conducted within a radius of 150 feet from the release area.
During this reconnaissance, no supply wells were found to be located within this radius.
Public water supplies are available to all of the properties within this radius.
On October 19, 2020 a licensed geologist from Paragon was on -site to supervise the in -
place abandonment of the 1,000 gallon heating oil UST performed by KBS Earthworks,
Inc. at 407 S. Ridge Avenue. A Letter of Abandonment documenting the in -place
abandonment performed by KBS Earthworks in contained as Attachment B.
Paragon certifies that the 1,000 gallon heating oil UST has been properly abandoned at the
subject site and that no water supply wells exist within a 150-foot radius from the source

On October 19th, 2020, KBS Earthworks, INC. performed an in -place abandonment of (1) 1,000-
gallon underground fuel oil storage tank located at 407 S. Ridge Drive Kannapolis, NC, using the following
method:
Excavated soils to access tank.
• Vacuumed all existing contents from within tank out.
• Properly cleaned (presser washed) and vacuumed additional contents.
• Filled tank in place using sand.
• Backfilled area to grade.
• Seeded and strawed all disturbed areas.
