Geological Resources, Inc. (GRI) presents the results of soil assessment activities conducted on April 9,
2018 at the above referenced site (Figure 1). The site is an active petroleum retail facility and
convenience store. Three registered underground storage tanks (USTs) (one 20,000-gallon regular
gasoline, one 12,000-gallon premium gasoline and one 10,000-gallon diesel) and 8 dispensers are
currently in use at the site. The soil assessment activities were conducted during the replacement of a
dispenser sump and associated product piping in the area of dispenser island 15/16.
GRI personnel arrived at the site on April 9, 2018. Gate Petroleum Company (Gate) personnel were
already on site and had disconnected dispenser island 15/16, removed the concrete, pea gravel and soil
over the dispenser sump and associated piping. Once, the product piping had been disconnected, the
old dispenser sump was removed. Additional pea gravel and soil was removed from the excavated area
to a depth of approximately 3.0 feet below grade. Pea gravel and soils that exhibited petroleum odors
were placed in 55-gallon drums for future disposal by Gate personnel. Perched water was noted in the
excavation. GRI personnel installed two soil borings in the area of the product piping adjacent to where
the dispenser sump had been previously located. Each boring was advanced using a stainless steel hand
auger and was advanced to a total depth of approximately 1.5 feet below the bottom of the excavation.
A soil sample was collected from each boring at a total depth of 4.5' below grade. The soil samples
collected were noted to be wet due to the infiltration of the perched water into each of the borings.
The soil samples (SS-1 and SS-2) were immediately placed in laboratory supplied glassware by GRI
personnel using clean, disposable nitrile gloves. The sample containers were immediately placed on ice
and subsequently transported to a North Carolina certified laboratory for analyses of gasoline -range
total petroleum hydrocarbons (TPH-GRO) by Method SW846 8015C and BTEX by Method SW846 8260B.
Based upon the laboratory analytical report, a detectable concentration TPH-GRO that exceeded the
Regulatory Action Level was reported in soil sample SS-1. Concentrations of benzene, toluene and

xylenes that exceeded the soil to water Maximum Contaminant Concentration (MCC) levels were also
reported in sample SS-1. A concentration of benzene that exceeded the MCC was also reported in soil
sample SS-2. The soil boring and sample locations are shown on Figure 2. The laboratory analytical
results for the soil samples are summarized in Table 1. A complete report of laboratory analyses of the
soil samples has been included as Appendix A. A disposal manifest for the affected soils removed during
the sump replacement is included as Appendix B. A UST-61 Form notifying the NCDEQ UST
Management Division of the release has been attached as Appendix C.
Based on this information, a soil quality violation exists at the site. The release should be reported to
the North Carolina Department of Environmental Quality by the Responsible Party (RP).

Tank: means the tank that stores the product and is part of the underground storage tank system
Piping: means the piping and connectors running from the tank or submersible turbine pump to the dispenser or other end -use equipment (Vent, vapor recovery, or fill
lines are excluded.)
Dispenser: includes the dispenser and the equipment used to connect the dispenser to the piping (e.g., a release from a suction pump or from components located above
the shear valve)
Submersible Turbine Pump (STP) Area includes the submersible turbine pump head (typically located in the tank sump), the line leak detector, and the piping that
connects the submersible turbine pump to the tank
Delivery Problem: identifies releases that occurred during product delivery to the tank. (Typical causes associated with this source are spills and overfills.)
Other: serves as the option to use when the release source is known but does not fit into one of the preceding categories (e.g., for releases from vent lines, vapor
recovery lines, and fill lines)
Unknown: identifies releases for which the source has not been determined
Definitions of Causes
Spill: use this cause when a spill occurs (e.g., when the delivery hose is disconnected from the tank fill pipe or when the nozzle is removed from the dispenser)

The date on UST-61 form is one month prior to me transferring to the UST section. Brett Morris vacated the position a
few months before I started. If the form was emailed to Brett, no one would have received it. Email me any additional
information that you have about the site (other than the UST-61). I'll try to get it reviewed and respond back by the end
of Tuesday next week.

Hi Trudy
I am contacting you concerning the Gate Petroleum #414 site located at 3699 Concord Parkway South in Concord,
Cabarrus County.
We conducted some soil sampling during a dispenser sump replacement at this site in April 2018. Soil contamination
was confirmed. We submitted a UST-61 form (see attached) to the NCDEQ-MRO on 4/27/18. To date we have not
received any response/NORR from the NCDEQ concerning this incident.
Gate Petroleum has decided to remove the UST system from the site in November of 2021.
Can you assign an incident number for this release? Once received, I will submit to you a UST-3 form detailing the
schedule for closure/removal of the UST system.
