The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Field 7, half of Field 11 & Field 15 have been sprayed/no ground cover at this time. Field 8 has been disced up.
The approved cropstand per the nutrient management plan shall be reestablished on these fields - which is
Field 7 (fescue) and Fields 11, 15 & 8 (small grains/millet). Ground cover issues with Fields 8 & 15 were noted
on the last audit of 3/23/2021.
2. Please submit a few page of the land application site logs for review.
3. Did not drive over entire site due to recent heavy rains. However, it appears that the required field posts are
present. The approved setbacks were not measured at this time, but appear accurate. No sign of land application
outside the permitted boundaries. No sign of heavy or recent application No odors noted.
4. The required signs are posted at the gated entrance and the access road is adequate.
5. Mr. Wallace stated that he will be attending a December continuing education class to obtain the required 4
hours of firm training & 3 hours of land app training.
