STATUS OF PERMIT:
Active; Permit to Construct (PTC); Permit to Operate (PTO) issued 12 July 2021. This permit shall expire 12 July
2026. Significant site changes may require permit modification. The owner or operator should submit a request to the
Section for permit review prior to 12 January 2026, and must update pertinent Facility plans including, but not limited
to, the Facility operation.

PURPOSE OF SITE VISIT:
Pre-operative Facility Compliance Inspection

STATUS OF PAST NOTED VIOLATIONS:
None applicable

OBSERVED VIOLATIONS:
There were no violations observed during this Facility Compliance Inspection. Please see the ADDITIONAL
COMMENTS section below for details concerning this inspection.

ADDITIONAL COMMENTS:
1) This Facility is a Landclearing and Inert Debris (LCID) Landfill and must conform to the operational
requirements of the NC Solid Waste Management Rules, 15A NCAC 13B .0566.
2) This Facility is permitted to receive land clearing waste; yard trash; untreated and unpainted wood;
uncontaminated soil; inert debris such as unpainted rock, brick, concrete, and concrete block; and asphalt in
accordance with NCGS 130-294(m).

3) The Facility is secured by means of a locked gate at the Facility entrance. The gate was closed and locked
upon my arrival and no attendant was on site during this Facility Compliance Inspection.
4) Proper signage including the Facility permit number, accepted waste types, and the emergency contact
number is posted at the Facility entrance.
5) The Facility access road is of all-weather construction and is being maintained in overall good condition.
6) Erosion control devices are in place and appear to be functioning properly. No off-site erosion was observed
during this Facility Compliance Inspection.
7) Edge of waste markers have been installed delineating the permitted waste disposal footprint.
8) This Facility maintains a Caterpillar D3G Crawler/Dozer on site for grading and Facility operations.

9) Construction of the Facility has been completed and appears to be incompliance with the approved
construction plan.
10) No violations were observed during this Facility Compliance Inspection.
11) This Facility may begin solid waste management operations in accordance with the Permit to Operate issued
12 July 2021.
