PURPOSE OF SITE VISIT: Compliance Inspection. STATUS OF PAST VIOLATIONS None. OBSERVED VIOLATIONS None. Page 1 of 3 NORTH CAR()LINAD_E Q�� OepaNnent W Envimnmenbl W.I� FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. ADDITIONAL COMMENTS: 1. 15A NCAC 13B .0566 (6), "An operator shall be on duty at all times while the site is open for public use to assure compliance with operational requirements and to prevent acceptance of unauthorized wastes." The hiring of additional employees may be required to properly manage the LCID. It is also recommended that a shelter is provided for the employee managing the LCID. Prior to the commencement of the inspection, Mr. Barnes stated that no waste has been placed in the LCID since a full-time attendant has not been hired. 2. The LCID has been mown with a brush hog and is ready to receive waste. 3. The roadway to the Phase I LCID cell has been completed to the turnaround at the end of the Phase I LCID cell. Based on the approved construction plans, a roadway is required to be constructed around the entire LCID. The roadway around the LCID will improve traffic flow and provide access for fire -fighting vehicles. This roadway must be completed around the LCID by December 31, 2021. It is recommended that the roadway be completed around the entire LCID, as per approved plans, to help with the flow of traffic. 4. The sediment basin to the northeast of Phase I of the LCID has been constructed. Three porous fabric baffles run the length of the sediment basin. The sediment basin also consists of a skimmer, concrete riser, and an emergency spillway. Vegetation has begun to grow on the side slopes of the sediment basin. Measures to monitor and maintain the basin should be put into place and followed. 5. During this inspection, the sediment basin and ditch were full of water. Water was exiting the outflow pipe slowly. Mr. Nichting stated that the skimmer would need to be inspected once the water has left the sediment basin. 6.
