John College - NCDEQ-DWM-SWS-FOB STATUS OF PERMIT: Active, Life of Site Permit. Issued April 10, 2019 and Expires 12/15/2055.
PURPOSE OF SITE VISIT: Partial Facility Compliance Inspection. STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: 15A NCAC 13B .0402 GENERAL REQUIREMENTS: (a) The owner or operator of a transfer station (site) shall comply
with the conditions of the permit issued by the Division. In the event of noncompliance with the permit, the owner or operator shall minimize the release of waste, leachate, or contaminants to the environment, and shall prevent adverse impacts to human health or the environment.
15A NCAC 13B .0405 OPERATIONAL REQUIREMENTS: (11) Site Cleaning and Maintenance: Unless otherwise stated in the site permit, all waste shall be removed from the tipping floor, the truck loading bays, and from behind push walls by the end of each day of operation.

ADDITIONAL COMMENTS:
1. Facility has new Life of Site Permit. Issued April 10, 2019 and Expires 12/15/2055. Reviewed Permit and Operations Plan. 2. Facility is permitted to receive MSW within Hertford County. 3. MSW is transported to East Carolina Regional Landfill in Bertie County permit number 0803-MSWLF-1993. 4. Facility has adequate signage. 5. Facility has fencing with locking gates for security. 6. Interior roadways are constructed of all-weather materials and maintained. 7. Upon arrival to this facility, John College observed that the tipping floor was full and waste was on the ground outside the tipping floor footprint and leachate containment. 8. Upon speaking with staff, the loader had been down since Friday (8/13/2021) afternoon and waste was still being received without removing waste from the tipping floor as required. 9. John College ask the facility to shut down incoming waste and to divert the waste straight to East Carolina Regional Landfill (permit# 0803-MSWLF) and/or to any available County Public Convenience site that is direct hauled to East Carolina Regional Landfill. This was done while on site. 10. At this point, John College asked Ms. Carolyn Carr to notify the Hertford County Manager (Mr. David Cotton) of the situation and ask him to come to the facility to meet with John College. Also, Ms. Judy Baker - Hertford County Solid Waste Manager was contacted while she was on vacation for input on this situation. 11. Mr. Cotton and John College met at the facility and discussed the Permit to Operate and the Contingency
Plan that were on-site. At this time, Ms. Ashley Jilcott, Sr. Administrative Assistant for Public Works came
to the facility to join the discussion of the Violations that were occurring at the facility. John College informed Mr. Cotton and Ms. Jilcott that there would be a Notice of Violation(s) sent to the Facility.
12. A mini excavator was brought to the site from the water department and all of the waste that was outside of the transfer station footprint was put in an empty container and pushed backed on the tipping floor. 13. There is a Public Collection Center onsite and managed well. Very Clean.
14. Screening records and incoming load records onsite and were not reviewed 15. Facility has 4 Certified Transfer Station Operations Specialist currently available. Judy M. Baker, expires 5/10/2022, Donnell Cooper, expires 11/16/2022, Alton Burke, expires 5/3/2024 and Carolyn Carr, expires
5/3/2024. 16. Windblown trash was not an issue during this inspection. 17. Digital pictures were taken during this inspection, and some are attached to this report.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

• Waste being removed from the ground and put into a container and on the tipping floor.
• Area on back push wall that is in need of repair. Bids have been submitted. Place a piece of plywood or something on the inside to keep waste from being pushed out onto the ground. Picked up daily, per Mr.
Cooper. No leachate noted.
