with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected vehicle tag#JP1850, VIN#1FDUFGTSKDA03612, this is not the VIN# listed on the firm application
– Ensure that the firm application this winter has the correct VIN# listed for this vehicle.
Tank appears structurally sound & securely attached.
Outlet covers were on and no drips/leaks were noted.
The lettering was correct on both sides of this vehicle per the information on the firm application.
2. A copy of the truck pumping logs were submitted by email later this day. It appears that the required
information is being maintained at this time.
3. Inspected the other permitted vehicle earlier this year. If you have not yet attended a 2021 continuing education
class then please register for one of the remaining class locations to obtain the required 4 hours of firm training.
