or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected vehicle tag#YA145471, VIN#5PVNV8JM054S51182 which is slightly different than the VIN# on the
firm application – ensure that the firm application submitted this winter has the correct info listed.
The tank appears structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted.
The lettering is correct on both sides of this vehicle per info on the firm application.
2. Reviewed the truck pumping log – the date of discharge & the discharge location is missing from each
pumping event, begin recording this required info. Discussed the required info to be maintained.
3. If you have not yet attended a 2021 continuing education class, please register for one of the remaining
locations to obtain the required 4 hours of firm training.
