PURPOSE OF SITE VISIT: Compliance Inspection STATUS OF PAST NOTED VIOLATIONS: None. OBSERVED VIOLATIONS: None. Date of Last Inspection: N/A The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. The access road to the facility is being managed properly at this time. 2. The facility is secured with a locked cable. 3. The tree limbs are tub ground by a contractor on an as needed basis and sent out for boiler fuel. The leaves are windrowed and turned. The leaves are utilized as a soil additive for Town projects and by local farmers. 4. It appeared that the facility may be approaching the 6,000 cubic yard amount. It was recommended during the site inspection, that the facility contact the grinding contractor to process the waste. Mr. Perkinson stated that he would measure the waste piles to determine the cubic yardage. 5. The tree branch windrow formed a semi -circle around the yard waste facility. It was recommended during the inspection, that the tree branch windrow be separated into smaller windrows, in order to provide fire breaks in the waste. 6. Permanent, clearly visible edge of waste markers must be established at the site, showing the less than two - acre operation area. Ensure that all materials are within the operation area. 7. The tree limbs and branch waste appeared to be clean. Several beverage containers were identified within the grass clippings. The unacceptable wastes were removed during the inspection. Ensure that the unacceptable wastes are removed from the waste stream. 8. Digital photographs were taken during the inspection.
