The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. This facility is a Type 1 Yard Waste Notification. Facility accepts land clearing debris consisting of stumps, whole
trees, tree limbs, branches, and untreated wood. Facility also accepts yard trash consisting of grass, leaves, brush,
shrubs, and other yard trimmings.

2. Land clearing debris is mulched and sold as mulch product.

3. Yard trash is burned and ashes are screened and used to make topsoil product sold. Facility has DAQ Air Permit
#0986R0Z for the use of one forced air curtain burner.

4. The facility can receive the aforementioned wastes for processing and storage in volumes not greater than 6,000 cubic
yards per quarter each year. The area of the waste processing and storage cannot exceed two acres.

5. No offsite sedimentation was observed.

6. An interior roadway has been made of all-weather construction and is being maintained in good condition.

7. There is at least a 25 foot access path around the site for fire control vehicles.

8. All setbacks are in place.

9. No standing water was observed during the inspection
