Box 453 Lemon Springs, NC 28335 FACILITY CONTACT NAME AND PHONE NUMBER: Name: Esther Saucedo Telephone: (919) 897-4231 Email address: greasenseptic@mail.com Fax: N/A FACILITY CONTACT ADDRESS: 567 Holder Road Sanford, NC 27332 PARTICIPANTS: Connie Wylie, Rousbel Saucedo STATUS OF PERMIT: Active – permit expires December 31, 2021 PURPOSE OF SITE VISIT: Routine STATUS OF PAST NOTED VIOLATIONS: N/A OBSERVED VIOLATIONS: 15A NCAC 13B .0836(a) “Each permit holder shall maintain a log that included the following information for each septage pumping event: (1) the date, type, quantity, and location of septage pumped; the location for tanks shall be a street address and the location for portable toilets shall be a route; and (2) the location of the discharge of the septage. Adequate truck pumping logs in accordance with the above rule were not available for review/are not currently being maintained. When this firm went into operation in July 2018 training/information was provided regarding the truck pumping logs as part of the requirement of having a septage management firm permit. Gave Mr. Saucedo an example log & discussed the required information to be maintained. Immediately begin recording the required info for each pump job (septic tank or grease trap) performed with this truck. On September 30 submit a copy of the truck pumping logs for August & September for review to determine compliance with the above Rule. Inspected pump truck Tag#YA143617, VIN# correct per the firm application. Tank appears structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted. Lettering is correct on both sides of this vehicle per info on the firm application. Some decals were damaged but are still legible at this time. 2. Mr. Saucedo stated that no one had attended a 2021 continuing education class yet. Register asap for one of the remaining class locations so that the required 4 hours of firm training are obtained. 3. Ensure that disposal authorizations have been submitted to the Raleigh Central office for any location that this truck goes to discharge septic tank waste or grease trap pumpings. Currently the 2021 firm permit only lists the Durham WWTPs & Grease Outlet. The Raleigh office did not receive a disposal authorization for Buffalo Lake WWTP. 4. Thank you for your time. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
