OBSERVED VIOLATIONS:
1. 15A NCAC 13B .0835(b) All permitted septage management firms shall display lettering on each side of every
pumper vehicle operated by the firm. The lettering shall include the firm name, town name, phone number and
septage management firm permit number as shown on the firm application. All lettering required by this Rule on
the pumper vehicle shall be no less than three inches in height and legible, distinguishable from the background
and not obstructed from view.
Inspected vehicle tag#YA131911. The town name (Sanford) was missing from both sides of this vehicle. Display
the town name (Sanford) on both sides of this vehicle with no letters less than three inches in height and
submit a photo showing this corrective action by August 31st. It was also noted that the firm name displayed
on the truck was “Maples Septic” and not Maples Septic Tank Service as listed on the firm application & 2021
firm permit. Also the phone number displayed on the vehicle is 919-258-3750, not the 919-356-5785 phone
number that is listed on the firm application. Ensure that the firm application submitted this winter lists the
correct firm name & phone number that are displayed on this vehicle.

ADDITIONAL COMMENTS
1. Inspected vehicle Tag#YA131911, VIN# correct per the firm application. The tank appears structurally sound &
securely attached. Outlet covers were on and no drips/leaks were noted. Lettering issues are noted on the front
page of this report.
2. Reviewed truck pumping logs – it appears that required information is being maintained except for the town
name of each pump job – begin including the town name for each job.
3. If you have not already attended a 2021 continuing education class to obtain the required 4 hours of firm
training then please register for one of the remaining locations asap.

or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
