The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected three vehicles for this firm permit
Tag#YA139312, VIN# correct per the firm application. The tank appears structurally sound & securely
attached. Outlet covers were on and no drips/leaks were noted. The lettering was correct on both sides of this
vehicle per info on the firm application.
Tag#EE2621, VIN# correct per the firm application. The tank appears structurally sound & securely attached.
Outlet covers were on and no drips/leaks were noted. The lettering was correct on both sides of this vehicle per
info on the firm application – however on the driver side the permit number & phone number were partially
hidden by the hose reel. Per Rule .0835(b) “All lettering required by this Rule on the pumper vehicle shall be no
less than three inches in height and legible, distinguishable from the background and not obstructed from view.”
On the driver side of this vehicle display the permit number & phone number (at no less than three
inches in height) in a different location where it is clearly visible. Submit a photo by August 31st showing
the corrected lettering.
Tag#YA162003, VIN#1HTMMAANXDH297634, capacity 2500 gallons - this is a new vehicle that was added
approximately a month ago/not currently listed on the firm application. The tank appears structurally sound &
securely attached. Outlet covers were on and no drips/leaks were noted. The lettering was correct on both sides
of this vehicle per info on the firm application.
2. Reviewed the truck pumping logs – it appears that the required information is being maintained except for the
town associated with each physical location. Begin recording the town name for the location of each pump
job along with the street address.
3. Please note G.S. 130A-291.1(h1) “A permitted septage management firm shall notify the Department within 10
days of placing a pumper truck in service that was not previously included in a permit issued to the firm and
shall make the pumper truck available for inspection by the Department. A septage management firm is not
prohibited from use of a pumper truck that meets the requirements of the rules adopted by the Commission prior
to inspection by the Department”. When adding new trucks in the future please ensure that the vehicle is in
compliance prior to putting a truck into service and notify the Department within 10 days so that an inspection
may be scheduled.
4. Mr. Bullard stated that he had attended a 2021 continuing education class.
