The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected pump truck Tag#YA139309, VIN# 1FDXK84E1RVA23442 (this is a different VIN# than listed on
the firm application – ensure that the firm renewal application this winter has the correct VIN# listed).
The tank is structurally sound & securely attached.
Outlet covers were on and no drips/leaks were noted.
The lettering is correct on both sides of this vehicle per the info on the firm application. However, some of the
decals were very damaged. Was informed that this vehicle will soon be taken out of service – if this does
not occur then these damaged decals need to be replaced.
2. Had inspected a new vehicle Tag#YA140158 last year on 3/13/2020. Have now been informed that another new
vehicle will be put into service in the coming weeks. Per G.S 130A-291.1(h1) please contact this office within
10 days of placing a new vehicle in service to schedule a truck inspection – the vehicle may be used prior
to the inspection if it is in compliance with the Septage Management Rules (lettering, tank integrity,
outlets cover etc).
3. Reviewed the truck pumping logs – it appears that the required info is being maintained except for the town
name. Begin recording the town name along with the street address for each pump job location.
4. If a company representative has not yet attended a 2021 continuing education class, please register asap for one
of the remaining class locations to obtain the required 4 hours of firm training.
