The Notification is described by the deed recorded in BOOK: 550 PAGE: 093. PURPOSE OF SITE VISIT: Compliance Inspection STATUS OF PAST NOTED VIOLATIONS None. OBSERVED VIOLATIONS None. Page 1 of 3 NORTH CAR()LINAD_E Q�� OepaNnent W Envimnmenbl Wel� FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. ADDITIONAL COMMENTS: 1. The overgrown vegetation, including trees along the eastern side slope, had been removed. Good Job! 2. Several pieces of uncovered inert debris were observed throughout the top of the LCID. If NCDOT wishes to no longer utilize the facility for the burial of LCID waste, NCDOT will need to follow the closure guidance identified in 15A NCAC 13B .0567. The disposal area shall be covered with no less than one foot of soil cover and sloped to direct the flow of surface water away from the landfill. The cover will also need to be stabilized with vegetation. 3. A tire was observed along the mown eastern side slope. Any unacceptable wastes need to be removed from the landfill and properly disposed of at a permitted facility. 4. No areas of standing water were observed during this inspection. 5. A sign is posted on the gate at the entrance showing the contact name and number in case of emergency. 6. The access road is of all-weather construction and appears to be properly maintained. 7. Edge of Waste Markers (EOWMs) had been installed and were identified during this inspection. 8. A large mound of soil that may be from ditch cleaning, was located to the west of the notified LCIDLF. No unacceptable wastes were observed in the soil pile during this inspection. Continue to remove and properly dispose of the unacceptable wastes from the soil on a frequent basis. 9.
