PURPOSE OF SITE VISIT: Follow-up inspection for 12/17/2020 NOV and previously observed violations. On
3/3/2021vehicles were inspected to confirm corrections of septage rule violations observed on pumper vehicles during inspections on 6/30/2020 and 12/07/2021.
STATUS OF PAST NOTED VIOLATIONS: On 12/17/2020 a 2007 International pumper vehicles operated by Bass Services was in violation of 15A NCAC 13B
.0835(a) for a leaking septage drain valve and 15A NCAC 13B .0835(b) for required town name lettering not meeting height requirements. On 3/3/2021: Lettering violations were corrected, no appreciable leakage observed at drain valve. On 6/30/2020 a 2004 Ford F-550 was in violation of 15A NCAC 13B .0835(b) where lettering indicating firm name was not as shown on the firm permit application. On 3/3/2021 : The firm name lettering on the truck matched the firm name on DWM records.
OBSERVED VIOLATIONS: No Violations observed.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

1) Lettering on the 2007 International included firm name, town name, phone number, and septage management firm
permit number and it was permanently attached. Lettering indicating town name was corrected to meet height
requirements of the rule, 3 inches in height or taller.
2) Lettering on the 2004 Ford F-550 did include firm name, town name, phone number, and septage management
firm permit number and it was permanently attached. Lettering indicating firm name was corrected to appear as shown on line 1 of the firm permit application.
3) The septage drain valve on the 2007 International was inspected for leaks. Some moisture was observed between
the cam lock cap and valve. No septage was observed leaking onto the ground or surfaces below the valve or cap.
