The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
On August 10, 2021, Chuck Kirchner met with Johnny Reynolds to conduct a pre-operational comprehensive inspection
of the 7716 PQR LCID Landfill. The LCID landfill is located at 7700 Pearman Quarry Road in Kernersville, NC in Guilford County.
1. The facility is a land clearing and inert debris (LCID) landfill owned and operated by Johnny Reynolds. 2. The facility permit, site plan, and operations plan were discussed. 3. The facility will be open to the public.
4. The facility is in operation Monday through Saturday 8:00 am to 5:00 pm or by appointment.
5. The facility is permitted to receive waste generated within Guilford, Randolph, Davidson, Forsyth, Stokes, Caswell, and Rockingham Counties, except where prohibited by North Carolina General Statutes Article 9 of Chapter 130A and the 15A NCAC 13B Rules.
6. The facility is permitted to receive land clearing waste, yard trash, untreated and unpainted wood, uncontaminated soil, inert debris, and (used) asphalt.
7. Landfill operations have not begun at this facility.
8. Per 15A NCAC 13B .0566(3), “Solid waste shall be restricted to the smallest area feasible and
compacted as densely as practical into cells.”
9. Per 15A NCAC 13B .0566(4), “Adequate soil cover shall be applied monthly, or when the active area reaches one acre in size, whichever occurs first.”
10. Adequate soils for cover shall be maintained on site as discussed in the field.
11. Excavation, grading, and fill material side slopes shall not exceed three to one (3:1).
12. Silt fence posts painted red are used as permanent edge of waste markers.
13. The facility annual report (FAR) shall be submitted to the Solid Waste Section by August 1 of each
year.
14. Facility is operating under a Guilford County Sediment and Erosion Control Plan, number 21-03-STOP-02900, issued March 18, 2021.
15. Erosion control measures were being installed during this inspection.
16. Continue to stabilize bare areas with vigorous grass cover.
17. No open burning of solid waste is permitted on site.
18. The owner or operator shall provide verbal notification to the Division of fires that occur at the site within 24 hours of the fire and shall submit written ‘Fire Notification Form’ to the Division within 15
days of the fire. 19. Access roads are of all-weather construction. 20. The facility is secured by a locked gate.
21. The facility has a proper sign.
22. The permit was recorded with the Guilford County Register of Deeds on August 10, 2021.
23. Prior to receiving permitted wastes for disposal at the 2-acre LCID, the permittee must submit a written
certification that is prepared by a Professional Engineer or Geologist, registered in the State of North Carolina to the Section to certify that the elevations of the landfill base has been constructed in accordance with those shown in the Site Plan – Final Conditions & Sections A-A & B-B the approved plan (FID 1587016) and Rules 15A NCAC 13B .0564(9) & .0565(a)(7)(K). The certification shall include a copy of
as-built survey drawings prepared, signed, and sealed by a NC Registered Land Surveyor. 24. Any changes to permit conditions, such as Permit Condition Attachment II, Part V, Number 3, section a, would require a permit modification. If this is desired, please contact the Solid Waste Section Permitting Branch to discuss a permit modification.
