The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected four pump trucks
Tag#HF3722, VIN# correct per the firm application
Tag#DF7211, mistake in VIN# on the firm application – it is 1FDXF47F92EB92969
Tag#JZ5604 (listed on application as Tag#EF7215), mistake in VIN# on the firm application – it is
3FRWF65H76V339822
Tag#JR2217 (listed on application as tag#DF7356), VIN# correct per the firm application
Ensure that the firm application this winter for the permit renewal lists the correct tag & VIN numbers
for each pump truck.
The tanks appeared structurally sound & securely attached on all four vehicles. Outlet covers were on and no
drips/leaks were noted. The lettering was correct on both sides of all vehicles except that the trucks say Siler
City & the office address on the application says Bear Creek – list “Siler City” on the application this winter.
2. Reviewed the truck pumping logs – it appears that the required info is being maintained. Ms. Hammer stated
that she had attended a 2021 continuing education class to obtain the required 4 firm hours.
