with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected pump truck tag#EV-1174, VIN# correct per the firm application.
2. The tank appears structurally sound & securely attached.
3. Outlet covers were on and no drips/leaks were noted.
4. The lettering is correct on both sides of the vehicle per info on the firm application except the phone number
displayed is 919-218-3380 which is not the number on the firm application. This was also noted on the
pre-operational inspection in 2015. Ensure that the firm application submitted this winter lists the phone
number displayed on the vehicle. Also the town name is faded on both sides of the vehicle but still legible at
this time.
5. Reviewed the truck pumping log – ensure that each daily porta john route has a “route name” listed and
that each septic tank job lists a full address (street address & town). Also Mr. Roundtree stated that he had
already attended a 2021 continuing education class to obtain the required firm hours.
