The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Reviewed the truck pumping logs & discussed the information to be recorded. It appears that the required
information is being maintained.
2. Inspected four vehicles at this time. The tanks appeared structurally sound & securely attached on all four
vehicles. Outlet covers were on and no drips/leaks were noted. In regards to the required lettering see below.
Unit #T04046, Tag#837165TL, VIN correct per the firm application. One of the numbers in the phone number
was missing on the driver side – replace the lettering and send a photo of the correction.
Unit #T04062, Tag#837166TL, VIN correct per the firm application. The lettering was correct on both sides of
this vehicle per info on the firm application.
Unit #T04128, Tag#826134TL, VIN correct per the firm application. The lettering was correct on both sides of
this vehicle per info on the firm application.
Unit #3144, Tag#66114P, VIN correct per the firm application. The wrong phone number (per the firm
application) was displayed on both sides of this vehicle. Display the correct phone number (800-871-3406) at
three inches or greater in height on both sides of this vehicle – send a photo of the correction.
3. It appears that there are errors with the vehicle list that was provided with the application this past winter for the
renewal of septage management firm permit NCS-00875. Also the permitted storage tank located at the
Fayetteville facility, SDTF 26-02, was not included on the firm application as a discharge location. Please
ensure in the future that the permit information provided is accurate & complete.
