The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. All photos were taken by Lee Hill on July 21, 2021.
2. The permit to operate expires on December 2, 2021. Facility staff were advised to contact Allen Gaither,
Permit Engineer immediately to start the permit renewal process. Mr. Gaither confirmed that facility staff
contacted him following the inspection to initiate permit renewal.
3. The facility is permitted to take land clearing waste, yard trash, untreated and unpainted wood.
4. The facility is secured by a metal gate and was locked upon arrival.
5. Signage is posted at the entrance to the facility showing the name, permit number, emergency contact
information and the types of material that are accepted.
6. The access road is constructed of gravel and was properly maintained.
7. No unapproved material was observed on the working face and the fill material side slopes did not exceed three
to one (3:1).
8. The working face of the landfill was less than one acre and facility staff stated that soil cover is applied monthly
as required.
