The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected two vehicles.
2. Tag#YA132942, VIN correct per the firm application.
The tank appears structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted.
The lettering was correct on both sides of this vehicle except the phone number displayed was not the phone
number on the application (it is the Elizabeth City phone number). Display the correct phone number (910-
875-7616 at three inches or greater on both sides of this truck and send me a photo of the corrected
lettering). The firm name displayed is “A Countywide Septic”.
Tag#YA142197 (listed on application as YA103857) VIN correct per the firm application.
The tank appears structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted.
The lettering is correct on both sides of this vehicle per info on the firm application. The firm name displayed is
“A Countywide Septic Service”.
3. Reviewed the truck pumping logs. The town name was missing on some of the pump jobs and also a
specific name for each WWTP used for the various jobs – begin recording more accurate info. Grease is
currently going to the Enviro Choice facility in Robeson County – it does not appear that the Raleigh Central
office received a 2021 authorization form for this disposal facility. Ensure that the Raleigh Central office
receives a current authorization to dispose of grease septage at Enviro Choice. Mr. Jones stated that he had
already attended a 2021 continuing education class in Shallotte. In regards to some discrepancies noted between
the lettering on the two vehicles & info on the current firm application ensure that the application this winter
for the permit renewal has accurate info for all items particularly in regards to the vehicles & the
lettering that is displayed.
