PURPOSE OF SITE VISIT:
Technical assistance – permit modification for higher application rate
STATUS OF PAST NOTED VIOLATIONS:
.0842(a)(17) & Permit condition 2 – the site has been harvested since the last site visit. Follow the nutrient management
plan in regards to harvesting the site in a timely fashion & rotating the fields in accordance with the 30 day wait.
Received additional field logs at this time – the monthly application rates have not been exceeded.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Did not inspect the whole site at this time, but the cropstand appears adequate and field markers are present &
no longer obscured by excess vegetation. Setbacks were not measured at this time. The required signs are
posted & no longer obscured by vegetation and the access road is adequate. During this visit no septage debris
was noted within the 500 foot radius on Field 2, near the two permitted storage tanks. Noted scattered areas of
weeds - was informed that weed control would be implemented soon.
2. Met to discuss a permit modification for a higher application rate. New fencing is going up to allow the cattle to
graze on the other fields – grazing info is in the nutrient management plan. Cattle shall only be allowed on a
disposal field once there has been the 30 day waiting period after the last septage application. Discussed the
current field ID’s (names) which could be adjusted in the new permit for the modification. Discussed what a
higher application rate may possibly be and discussed the current monthly & yearly application rates for the
fields and for the entire site. Reviewed the items needed for a permit for a higher application rate such as a new
SLAS application, a revised nutrient management plan, public notice for 30 days, waste analysis samples and
certification in the Land Residuals Course at NC State.
