The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Met to discuss a future permit modification from a storage facility to a treatment facility. Additional tanks &
box are on site in the new area for the treatment facility. The additional tanks & box are empty and are not
permitted for use at this time. Discussed changes to the basic site drawing that was previously submitted – now
2 frac tanks will be used instead of 5 horizontal tanks of various sizes. The two frac tanks shall be located at
least 200 feet from the creek (the distance was previously measured). If additional area is needed for the
treatment facility then the permitted boundary/fence of the disposal field behind the facility shall be adjusted.
Discussed the items needed for this permit modification to a treatment facility – a new SDTF application, final
site drawings with a PE stamp, a signed operation manual, a leak test (signed by a PE) for any buried tanks or
lines and a landfill or compost authorization for the disposal of solids (the effluent will go on the permitted
fields). Treatment logs will be required when the permit modification is issued and authorization forms for any
firm other than Belton’s Septic Tank Service that may utilize the facility.
2. The two currently permitted storage tanks appear structurally sound. These are the only tanks on site that may
be used for the storage of septage at this time (until a new permit is issued to include any other tanks). It appears
that septage is being transferred to/from the facility in a safe and sanitary manner. No spills or leaks of septage
noted. No apparent odors or vectors. The approved setbacks are being met. The required 5 foot cleared radius
(free of excess vegetation or debris) is present around these two tanks at this time. The required signs are posted
and the access road is adequate.
