OBSERVED VIOLATIONS:
G.S. 130A-291.1(h1) A permitted septage management firm shall notify the Department within 10 days of placing a
pumper truck in service that was not previously included in the permit issued to the firm and shall make the pumper truck
available for inspection by the Department. Was informed during this inspection that a new vehicle was put into
service early this year without the Department being notified. Any new vehicle added in the future requires the
Department to be notified within 10 days and the vehicle may be put into service prior to an inspection if it is in
compliance with the Rules (regarding lettering, tank, outlets etc).

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected the following vehicles at this time.
Tag#YA161930, VIN#2NP3LJOX7MN765912, 3500 gallons – this is the new vehicle (shown above) and
which was not listed on the recently submitted firm application.
Tag#JP8093 (previously was tag#JP8275 as listed on the recently submitted application) VIN# correct per
the firm application.
Tag#JM3341, VIN#1FD0W5H71KED01212, 700 gallons – this vehicle was not listed (tag or VIN#) on the
recently submitted firm application.
The tanks appeared structurally sound on these three vehicles & securely attached. Outlet covers were on and no
drips/leaks were noted. The lettering was correct on both sides of all the vehicles per the info on the recently
submitted application. Was informed by Mr. Shingleton that vehicle Tag#YA128341 listed on the firm
application was currently out of service and not available for inspection at this time. Mr. Shingleton also stated
that a different tank would be installed on vehicle tag#JP8093 (which will not affect the lettering which is
currently on the doors & side rail) – Mr. Shingleton is to send a photo when the tank is switched out.
2. Reviewed the truck pumping logs at this time. The gallons for each pump job and the full physical location were
missing from the recorded info (which were manifests for the Anson County WWTP). Discussed the required
information to be maintained and gave Mr. Shingleton an example truck log - begin recording all the
needed info. Mr. Shingleton still needs to attend a 2021 continuing education class before the end of this year –
instructed Mr. Shingleton to register for a class location remaining in 2021.
3. The firm application that was recently submitted had the wrong permit number on the application and
also incorrect or missing info for the vehicles. Please ensure that the firm application submitted at the
end of this year has the correct information. The Raleigh Central office has not issued the 2021 firm permit
yet and is currently waiting on a WWTP authorization form. Please submit authorization forms to the
Raleigh Central office for any WWTP that may possibly be used in 2021. Please contact me if you have any
questions or concerns regarding this inspection report.
