OBSERVED VIOLATIONS:
15A NCAC 13B .0835(a)(2) Vehicles used for the transportation of septage shall be operated and maintained to prevent
leaks and spills of septage and all valves shall be in working order and be closed during transportation.
At the time of this inspection vehicle tag# JP8035 had a steady drip from the valve outlet area. Instructed Mr. Bryant to
place a bucket under the outlet to catch the leak and take this vehicle out of service until the problem was corrected. Mr.
Bryant stated that he would repair the outlet as needed and send me a photo to confirm that the corrective action
was taken.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected two vehicles
Tag#JP8035, VIN# correct per the firm application.
Tag#YA161939 (was previously tag#JP8047), VIN# correct per the firm application.
The tanks on these two vehicles appeared structurally sound & securely attached. Outlet covers were on – a drip
was noted on vehicle tag#JP8035 but no drips/leaks were noted on vehicle tag#YA161939. The lettering was
correct on both sides of both vehicles per the info on the current firm application. However, some scattered
decals (on both vehicles) were damaged and will need replaced in the future – still legible at this time.
2. Reviewed the truck pumping logs – it appears that the required information is being maintained. Mr. Bryant
stated that he had already attended a 2021 continuing education class.
