PURPOSE OF SITE VISIT: Comprehensive Facility Compliance Inspection. STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS: 01 - Life of Site Permit issued April 08, 2018. 02 - Facility has a locking gate and fencing for security. Entrance signage has required information. 03 - Roads are constructed of all-weather construction. 04 - New Landfill directional signs have been installed and looks good. Some Edge of Waste Markers to be replaced. 05 - Electronics have been wrapped and on pallets for removal. 06 - Scrap tires are being laced into a covered box trailer. 07 - White Goods & Scrap metal area is being managed well. 08 - This facility has 6 Certified Landfill Operators: Justin Harrison, expires 9/27/2022, Johnny Rascoe, expires 12/6/2021, Kimley Brown, expires 9/27/2022 William Brown, expires 8/21/2023, Elizabeth Daniel, expires 8/21/2023 and Ronald Brownfield, expires 10/19/2022. 09 - Non-conforming waste was not an issue at the C&DLF working face. 10 - Facility is using the S/M approved cover at the 50/50 rate. 11 - Small amount of windblown trash to be removed by the end of the working day. 12 - Screening records were reviewed and were found to be up to date. 13 - Cover records were reviewed and up to date. Be sure to keep a record of incoming cover materials as required by your permit to operate: page 7, #16. C. 14 - Incoming load receipts have required information. 15 - Stormwater sampling results reviewed. Reported on 2/23/2021 & 5/25/2021. 16 - Landfill Gas Monitoring reports were reviewed. Report was dated 2/25/2021 & 5/18/2021. 17 - Facility is getting to start to add in the approved area on the East side of the landfill. A drainage swell will have to be re-routed. To do this, the current area where the Yard Waste is located, is the relocation area for the swell. 18 - Martin County owns an adjoining tract of land beside the permitted Landfill. 19 - Currently, there is an open field not in use for agriculture. Sherri Stanley (NCDWM-Permitting) discussed the potential of adding a Yard Waste Notification there to handle the movement of the Yard Waste area on the landfill. 20 - During this inspection, the proposed Yard Waste Notification area was evaluated by John College & Justin Harrison. Site will meet the requirements set forth by DWM to use. Justin Harrison to get needed information an apply for this Yard Waste Notification. 21 - Covid-19 recommendations were followed during this inspection. 22 - Digital pictures were taken during this inspection.
